Alarmingly, 80-90% of patients in addiction programs relapse after discharge, often due to disengagement and lack of motivation. This statistic underscores a critical gap in aftercare support and highlights the urgent need for innovative solutions.
Konain Qurban, Founder and CEO of Axon Health, joined the CHEO Research Institute’s Core Innovation Invention Studio Session in 2025 with a clear understanding of the problem, but through the studio session’s structured support and mentorship, he transformed that understanding into action, extracting critical insights from frontline conversations and sharpening his messaging for decision-makers.
“The program was a game-changer,” said Konain. “Rather than asking what I needed, they identified it and guided me through the process with expert support. It connected me with a community of like-minded innovators who truly understood the journey.”
Feedback from over 50 staff across addiction treatment centers and recovery homes in North America revealed the real pain points: overwhelming casework, fragmented systems, and limited capacity for personalized outreach.
To tackle these issues and with the help of Core Innovation, Axon Health developed a secure, AI-powered data collection and reporting platform that integrates multiple tools, electronic medical records, CRMs, messaging apps, and payment systems, into one streamlined interface. This consolidation reduces administrative burden, improves data accuracy, and enables aftercare teams to deliver more personalized, timely support to clients in recovery.
Axon Health’s platform has delivered measurable improvements:
- 90% reduction in time spent on administrative tasks
- Enhanced ability for staff to maintain personalized connections with discharged patients
- Increased engagement and motivation among patients during the transition to long-term recovery
These outcomes directly address the root causes of relapse and demonstrate how targeted innovation, supported by expert guidance, can lead to meaningful change.
Building a resilient ecosystem for mental health and addictions care
To date, Axon Health has secured its largest contract, spanning across 4,000 homes in North America. Actively working with four addiction treatment centres and recovery homes in Ontario, the platform continues to evolve based on real-world feedback, with a long-term vision to unify the global mental health and addictions ecosystem into a coordinated, data-driven system of care.
